Job Description for Case Manager
The Case Manager facilitates continuity of care by ensuring that the needs of the patients, including observation patients are recognized and address through the coordination with other health care team members to bring about the best possible cost effective quality and clinical outcome. Reviews patient admissions within one working day to determine appropriateness of hospitalization utilizing intensity of service and severity of illness criteria and continues daily reviews throughout stay assuring patient's need for acute level of care.
Job Requirements for the Case Manager
Must possess a minimum of two years full-time direct care experience and/or hospital based case management experience 2 years.
A graduate from a school of nursing Bachelor degree in nursing preferred.
The Case Manager must be proficient with Microsoft Word, Excel, PowerPoint and Access; Must have excellent planning and organizational skills; The Case Manager must have excellent written and verbal communication skills; Must have satisfactory work processing skills; Must have the ability to work independently, with little or no supervision required.
